Player Waiver Form

2022 Indoor Turf Season:

  • To register a team, please send an email to southcentralsoccerleague@gmail.com with the following information: Captain’s first and last name, team name, color, logo (please attach a file with logo) and include the first name, last name and age of all player that will register for your team. Your team registration will only be completed when SCSL has received the full payment of registration from the captain. The cost per team for the 2022 indoor season is $700. SCSL is not responsible and will not accept payment from individual players.
  • All games will be played in Winkler, MB at Meridian Exhibition Centre indoor turf field. Games will be on Tuesdays and Thursdays from 8:00pm-11:00pm with 2 games per night. There are 8 teams in the league and each team will play 7 games.
  • Captains must provide game sheet filled out before every game with all player registered to the team and give it to the referee. The captain must mark the player that will play that game and mark which player are absent. Coaches must be noted on the game sheet as well.
  • The latest FIFA Rules will be used (with few exceptions). Find all FIFA rules here: IFAB Laws of the Game
  • Shin Pads are required.
  • 3 Yellow Cars accumulated during the season will result in a 1 game suspension.
  • Red card is a minimum of 1 game suspension. Depending on severity more game can be added to the suspension and will be determined by the League.
  • Matches will be 7v7 players. Game time is two halves of 35 each minutes with 5 minutes halftime break. There will be 10 minutes of warm up before the game.
  •            First Game: 8:00 warm up of 10 minutes; 8:10 1st half of 35 minutes; 8:45 halftime of 5 minutes; 8:50 2nd half of 35 minutes; 9:25 game ends sharp.
  •            Second Game: 9:25 warm up of 10 minutes; 9:35 1st half of 35 minutes; 10:10 halftime of 5 minute; 10:15 2nd half of 35 minutes; 10:50 game ends sharp.
  • Everyone need to be out of the building by 10:55 pm sharp!
  • There will be no time added at the end of the game under no circumstances. In case of delayed start or injury, the game will only be played till the indicated time above and not any longer.
  • For a match to start, both teams must have a minimum of 5 players. If one team is short players or player that are running late, the other team can still have a full team of 7 on the field. As long as the team has 5 player the match must go on.
  • If a team does not have enough players to start the match on time, they will be provided a grace period of 5 minutes. Once the 5 minutes are up and the team still does not have enough players to start the game, the referee will call the game and it will result in forfeit loss of 0-3 for team.
  • Unlimited substitutions per game, players have to be of the field to sub on/out. Substitutions can only be made in the team bench area.
  • 15 players per Team for the season. No Substitute player will be allowed (that are not signed up to a team). No player under the age of 16 will be allowed to participate. Players under the age of 18 must provide a printed and signed copy of the Release of Liability for Minor Participants Form to be able to play. The form must be signed by Parent or legal Guardian.   • No more than 4 players from STORM/RIOT per TEAM throughout the Season (even if player joins STORM/RIOT later in the season).
  • Players must submit registration form 72 hours prior to game to be eligible to play. A player who participates in a game when they are not eligible to play, will result in a forfeit for the team (0-3 loss).  Each player will receive a confirmation email of successful registration. Check spam/trash email.
  • Each team can register 2 coaches. Only registered coaches and players will be allowed to be on the bench and must be on the game sheet.
  • No fans allowed on the bench and field at any time. No fans on the field/bench during warmup, halftime and after the game.  NO FOOD or COFFEE on the Turf. Only WATER
  • Each team will be required provide 2 referees.

Outdoor Season:

  • Game time is two halves each 25min with 5min halftime break
  • Unlimited substitutions per game, players have to be of the field to sub on/out. Substitutions can only be made in the team bench area.
  • Captains will provide game sheet filled out with all player as submitted  beginning of the season
  • 14 player per Team. No Substitute player will be allowed (that are not signed up to a team). No player under the age of 16 will be allowed to participate. Player must be 18 years and older. Players under the age of 18 must provide a printed and signed copy of the Release of Liability for Minor Participants Form to be able to play. The form must be signed by Parent or legal Guardian.     WAIVER INFORMATION FORM

    1. NATURE OF ACTIVITY - Outdoor/Indoor Soccer

    - I, the Re-leaser, wish to participate in the following Activity . I acknowledge that I am of legal Age required to do so:

    South Central Soccer League based in Winkler, MB

    2. WAIVER

    - I, the Re-leaser, hereby waive, release, and discharge the Releases (who runs and/or operates this Activity) and his or her Heirs, legal Representatives, Administrators, and Executors for all Liability for or by Reason of any Damage, Loss or Injury (including Death) to myself or my Property which has been or may be sustained as a result of my Participation in the Activity, and notwithstanding that such Damage, Loss or Injury may have been caused solely or partly by the Release's Negligence.

    3. ACKNOWLEDGEMENT

    - I, the Release, hereby acknowledge the following:

    - 1.01 I have read this Release thoroughly and I fully understand it.

    - 1.02 I am voluntarily executing this Release, and would not otherwise be permitted to participate in this Activity.

    - 1.03 This Release restricts me from suing or otherwise claiming legal Action against the Releasee, presently or at any future time, for Damage, Loss, or Injury that may occur to myself or my property as a result of participating in this Activity.

    - 1.04 I have been granted the Opportunity to seek independent legal Advice before executing this Release.

    4. BINDING EFFECTS

    - This Release shall be binding on and inure to the Benefit of the Spouses, Heirs, Executors, Successors, legal Representatives, Administrators, and assigns of the Releaser and Releasee.

    5. MEDICAL CONDITIONS

    - To the best of my knowledge, I, the Releaser, am in good Health and do not have any Illnesses, Ailments, physical or mental Disabilities that would limit or prevent my Participation in this Activity. I will provide a medical Examination and Clearance if required.

    6. FINAL AGREEMENT

    - This Waiver and Release contains the entire Agreement between the two parties, and there are no further Provisions, either written or oral. The Terms of this Waiver and Release are contractual and not a mere recital.

    7. GOVERNING LAW

    - This Waiver and Release is being delivered in, and shall be governed, construed, enforced, and interpreted by, through, and under, the Laws of the Province of Manitoba, excluding Conflict of Law Principles that would cause the application of Laws of another Jurisdiction.

    - players under 18 have to bring a parenting consent Form singed from parent or legal Guardian.
    -I understand that athletic participation comes with the risk of injury or getting sick in any matter.
    -I accept all liability for any damage to the the playing equipment caused by my child careless, negligent and/or improper handling.
    -I accept sole responsibility for my child personal possessions and athletic equipment.
    - Include Name from parent or legal guardian signature, Address and contact info, signature from team captain.
    with out the Form a under 18 will NOT participate in the League.

    9. PUBLICATION

    - I, the Releaser, fully understand and agree that my Name and Photo may be made public on Websites and/or Social Media Accounts in Association with South Central Soccer League for entertainment, promotional and educational Purposes.

    - The Associates shall not be held liable, when Damages and/or Injuries (including Death) to Registrants who intentionally provide false Information (e.g. false Date of Birth) about themselves, occur.

    - All Games of Teams accepting Individuals named above shall be forfeited until a signed Consent Form is handed in or the Individual reaches the Age of Majority.

    Players: Only register once your captain has signed up your team.
